Assignment 3 Please use the grading rubric to create an outline of your assignment. Each section of the rubric should be a section of your final paper and could become the headings. Your assignment will be graded based on each element of the rubric. Compare each section of your paper with the rubric to ensure all elements are covered. LifeCareWriter is an example of a software program that helps case managers plan care for their patients. You may use LifeCareWriter free for one month. For Assignment 3, you will create a fictitious patient with your chosen chronic illness, and use the LifeCareWriter to create a discharge plan for that patient. Your paper will begin with an introduction. To address the first section of the rubric, you will cut-and-paste your assessment guide (the Life Care Plan Summary and Report). To meet the second section of the rubric, you will cut-and-paste the Life Care Plan of care (the tables in LCW). The third section of the rubric will be your interpretation: What did you learn from using the LCW? What do you think about using this tool? Did it help you to consider areas you might not have previously included? You will then write a conclusion. Please follow the example paper provided for your guidance. |
