In this assignment, based upon your chosen chronic illness from Assignment 1, you will be exploring the use of evidence-based practice and how it is incorporated into case management patient care guidelines and pathways. Identify and discuss current best practices, published guidelines and pathways for your chosen chronic illness, addressing the importance of basing our case management on current evidence.
Provide support from a minimum of two scholarly sources. Paper should be 3-4 pages in length.
