The health promotion plan should be completed on an individual identified from your
clinical practice, student, or professional. It is recommended that you choose someone who is not a
close friend or family member to do a health promotion plan. The purpose of the assignment is to
complete a health promotion plan that is based on the individual’s health risks and needs. Some
risks are related to age and gender, while others are related to environment, occupation, and health
values. As a result, a health promotion plan is individualistic. Some recommendations are
appropriate for everyone, some specific to each person. A health promotion plan is only beneficial if
the client sees it as beneficial, which means it must be negotiated with the client. That could mean
that the final plan does not include all of the recommendations that the health care provider would
like to include or some may be adjusted to meet the client’s lifestyle. The negotiation process is an
important part of health promotion and should be included in the completed plan.
The guidelines presented here outline the information required to complete an individualized
health promotion plan, the health promotion recommendations you identify, the negotiation process,
and the final negotiated plan. All points should be included in your final paper.
In order to complete the HPP it may require 2 visits with the client. The first visit is for data
collection. The second visit is to present your plan and with the client develop the negotiated HPP.

7. What types of screening tests have been done in the past 5 years i.e., B/P, pap test, mammogram,
PSA, Cholesterol, blood glucose? Results if can recall?
8. What are leading causes of death/major health concerns for this age group (also take gender and
race into consideration)?
9. Based on the data you have gathered, what is your assessment of health risks for this client?
10. Present your proposed health promotion plan. Include the areas of health promotion, protection,
and preventive services.
11. Discuss negotiation of your plan with client taking into account what client values and aspects
of plan client is willing to adopt. Education and mutual goal setting is a part of this process. In the
end, it is the client’s decision and the client’s plan.
12. Present final negotiated plan agreeable to client.
